Request Your Medical Records

Our Release of Information staff will be happy to assist you with requests for your medical records. We also assist providers with completing forms for patients.

We can assist you with the following —

  • access to your medical records
  • obtaining copies of your medical records
  • requests to amend your medical records
  • completion of forms for benefits, insurance, and other reasons
  • assist with MyHealthevet In Person Authentication (IPA) process

The Release of Information Staff is expert in our patients' rights and their medical records.

The Release of Information Office is located in Building 200, Ground Floor, Room E013.

How to Request Information

To request a medical record, please complete one of the forms listed in the column to the right, sign the form, and mail it to the following address —

Because forms must contain an original signature, e-mailed forms cannot be accepted.

Requests for records will take approximately 10-14 days to process.

On-Line Access to your personal health information: My HealtheVet

My HealtheVet ( provides access to various information in your medical record via any computer. Your data is password protected and uses secure servers. To retrieve data, review labs, view provider notes, check appointments, refill prescriptions on-line, and send a secure message to your healthcare team, you must (1) register from any computer and (2) come in-person to a VA facility to be authenticated.

My HealtheVet In-Person Authentication (IPA) (Upgrading to a premium account)

To complete the IPA authentication process you must appear in-person at the Release of Information desk at the Togus VA Medical Center or at any VA Maine Primary Care office at Togus, Saco, Portland, Rumford, Lewiston, Bangor, Lincoln, Calais or Caribou.  This is a two minute process and no appointment is necessary during normal business hours.

You must present a one picture ID such as a VA card, driver's license, passport, or retired armed forces ID. Complete Individual's Request for a Copy of Their Own Health Information (VA 10-5345a-MHV) listed in the column to the right.

An individual requesting a copy of records or information under Rights of Access will be provided the first copy of the requested record or information free of charge.
